randomizing Medical History Permissions

Okay so Simon will be wanting to talk to people about their medical history whenever he can track them down. It'll be brief and can (and probably should, unless you have a specific idea) be handwaved, but if your character is willing to sit down with him IC, there's a handy little form here for the info he'd want.

<b>Character:</b> Who are they? <b>Canon:</b> Just for my reference :D <b>Are they human?:</b> And how human! Do they have a metahuman healing factor or need a Kryptonite needle to pierce their skin? <b>Long-term Medical Conditions:</b> Simon also will ask about mental illness but he's not the most sensitive person so if your character would lie about that part, that's cool! Just make a note. <b>Special Considerations:</b> Anything from 'allergic to peanuts' to 'exposure to sunlight causes me to burst into flames' can go here <b>Volunteer Status:</b> Are they open to the idea of helping the doc out on short notice? Are they qualified to do so? <b>Emergency Contact:</b> If your character would like to leave the medical decisions in case of their incapacitation to anyone in particular, Simon will try to respect that. If not, he'll do his best to make the decisions on his own.
